Holiday Request Form

Contractors: All contracted staff must make a request for holiday that they wish to take. Please submit your holiday request using this form. Once it has been authorised you will be contacted to advise you that your holiday has been approved, and to give you a summary of remaining holiday entitlement.

Sub-Contractors: Subcontractors are not required to advise us of any leave that they wish to take. However it can help with planning if we know that sub-contractors are going to be absent for extended periods of time. You can use this form to advise us of planned absences if you wish.
